INSTRUCTION FOR COMPLETING

THE 2011/2012 FU OTUOKE ONLINE CLEARANCE

Please go a to reliable Internet Centre that has knowledgeable staff to process your clearance. Neither FU Otuoke nor its agents will be held liable for any errors or non-receipt of documents purportedly uploaded.

It is YOUR RESONSIBILITY to ensure that all filled-in details are accurate.

A.     FOLLOW THIS PROCEDURE:

         1.  Buy an FU Otuoke Acceptance e-Pin Slip from Diamond Bank Yenagoa.

         2.  Go to http://www.fuotuoke.ac and click on (Admission/Clearance Procedures) or http://www.fuotuoke.ac/adminclear/

            3.  click on (Step 2: Online Clearance)

         4.  Your existing details filled in the Post-UTME will be used. Any errors noticed by you can only be corrected through the Admissions Office.

         5.  You will need to fill in any required details in the empty spaces provided. Ensure you provide accurate and up-to-date information.

         6.  You will need to scan and upload the documents required in Part B  below. To ensure a speedy clearance verification process, ensure that all scanned and uploaded documents are clear and are of the right size.

         7.  To verify that your online clearance has been properly done, ensure that you preview your filled-in details at the last stage of the clearance wizard. Effect any corrections now, as making corrections later is a process and will be difficult.

         8.  When done, please proceed to the Registrars Office @ FU Otuoke for documents verification by the FU Otuoke Admissions Office. Physical verification of documents will begin on Monday March 05, 2012.

Please bring along original copies of the same set of documents used for online clearance, as any discrepancy could cost you your admission.

Also bring along a copy of your Post-UTME Application Form.

B.     DOCUMENTS REQUIRED:

All documents should be scanned in the attached in JPEG/JPG format.

         9.  WAEC/NECO/SSCE/GCE/ (O/L) NABTEB result. You are allowed a maximum of two (2) results.

         10. Signed Statutory Declaration

         11. Two (2) Signed Attestation letters addressed to:

            The Registrar,

            Federal University, Otuoke,

            Ogbia,

            Bayelsa State. Nigeria

         12. Birth Certificate/Age Declaration

         13. Local Government Area Identification Letter.

C.     DOCUMENTS REQUIRED FOR VERIFICATION AT THE ADMISSIONS OFFICE FU OTUOKE:

         14. Original copy of this Admission Letter

         15. Signed Acceptance Form

         16. All original documents used for the Online Clearance, (step B above).

         17. Printed Original JAMB Admission Letter from JAMB website.

NOTE:

You are NOT to pay school fees until you have done your online clearance, verified your documents by the Faculty Officer of your Faculty at FU Otuoke and have been told to go and pay your fees. School Fees are to be paid at Diamond Bank Yenagoa Branch.
